| IPS is a wholly owned subsidiary of Discount Drug Mart, Inc. headquartered in Medina, Ohio. Mail order often provides additional savings with the convenience of home delivery. |

| How does a member use the mail service? |
 |
| Ask your physician to prescribe needed medications up to a 90-day supply, plus refills. If you receive a prescriptionfor a new medication, ask your physician to write two prescriptions, one for a sufficient supply of your prescription that can be filled at a local pharmacy to cover immediate needs AND one for a 90-day supply with refills. |

| How does a member register for mail service? |
 |
| The preferred way to register for the mail order is to complete a mail order brochure and fill out the Patient Profile information and mail it in with your new prescriptions. |

There are 3 ways you may obtain the IPS brochure form to register:
- You may download the IPS mail order brochure and print it out to mail in with your prescription(s).
- You can also register or enter your profile by using the online process via the IPS website at www.ipsrx.com.
- Another way you may obtain a brochure for a patient profile is calling IPS. Doctors can call in (1-800-233-3872) or fax (1-440-930-5540) prescriptions. When a doctor calls a prescription in, IPS will try to find the patient by name, DOB and/or phone number. If they are unable to locate a patient, IPS will call the patient. If they cannot reach the patient, they will call the doctor back.

| Once you have filled out or registered for the mail order, include your payment method (see registration for forms of payment). Upon completing and establishing your account, you then may mail in prescriptions from your doctor or have your doctor fax your prescriptions to IPS to be filled. |

| The medication will be shipped at the expense of IPS using the United States Post Office PRIORITY MAIL. |

| How long does it take to get my order through the mail service? |
 |
| Your order will be processed within 48 hours after IPS receives your order. Your medications will be returned via UPS or US mail along with the reorder instructions and return envelopes. This turnaround time is usually 7-14 days. |
